JOB DESCRIPTION:
Medical assessment of client as part of the intake process,
which includes medical intake, discharge staffing, securing equipment needs,
in-home nursing and documentation of foster parent medical training prior to placement.

Participation in the development and review of client service plans and supervises,
in conjunction with medical director and physicians, the medical components of the plan.
Provide technical assistance to coordinators and Mentors regarding the medical care/home care
issues along with operation and maintenance of durable medical equipment.

Attend school, in home nursing, IL Mentor, IEP and court staffings as necessary.
Generate Psychotropic Consents and develop a monitoring system.
Carry pager to assist with emergency coverage and support for all clients
by on call rotation. Maintain a system of Quality Assurance for the program
including but not limited to: utilization of the medical database for quarterly reviews of caseload,
coordination of reviews by medical director, quarterly supervision of medical coordinators,
implementation of corrective plans as needed, and chart audits.

Advanced Practice RNs will demonstrate clinical expertise and an in depth
knowledge of inpatient and outpatient care activities; consult with other
professionals/customers; develop appropriate educational programs for
patients, families and staff; mentor staff in aspects of advanced patient
care; and actively participate in research pertinent to area of expertise.

MSN; current IL RN & APN licensures; NP certification by a national certifying
body as approved by the IL Department of Regulation APN board; and ACLS are
required. CVT experience also essential. Two-to-five years experience in
LVAD CVT heart transplant cardiac surgery preferred.
